Application Process

Veterans may use the following link: https://www.va.gov/health-care/how-to-apply/ Our routine process for getting Veterans vetted through VA health care services is for the Veterans to meet with a local Health Benefits Advisor. For our Clarksville Hub and Spoke sites, Ms. Regina Moore is our primary Health Benefits Advisor. Veterans can meet with her in person most Mondays, Wednesdays, and Fridays at the Clarksville VA Clinic located at 782 Weatherly Drive, Clarksville, Tennessee, or most Tuesdays and Thursdays at the Fort Campbell VA Clinic located at Blanchfield Army Community Hospital, 650 Joel Drive, Fort Campbell, Kentucky from 7:30 a.m. to 4:00 p.m. In the absence of an available Health Benefits Advisor, an employee can provide the Veteran with a hardcopy of the VA Form 10-10EZ (attached) to complete and return with ink signature. The VA employee can then scan the documents and forward it to the local Health Benefits Advisor to initiate the process.
