About

Our agency provides hospice care for those 18 years of age and older with a life limiting illness and a prognosis of 6 months or less. Hospice services are provided in the comfort of a patients home or in a skilled nursing facility. Services include but are not limited to skilled nursing care, certified nursing assistants, social worker, chaplain and volunteer services all of which are overseen by our Medical Director, Dr. Nikkalyn DeLaurentis. We also provide medical equipment to assist in the home, medications, and many medical supplies to provide the best care for our patients. In Hospice care is covered by most all insurances.

Our team assists with Advanced Directives, Advocacy, Caregiver Resources, Community Outreach, Education, Pian Management, Respite Care, Spiritual Support and much more.

Eligibility Requirements

Hospice care is for people who have a life-limiting illness with a life expectancy of six months or less, and have chosen to focus on comfort care over treatment. A physician must write 'orders' for hospice care. We work closely with a patients primary care provider to obtain necessary documents and orders to begin our services.
