Gentiva Hospice (formerly Avalon Hospice)

Our agency provides hospice care for those 18 years of age and older with a life limiting illness and a prognosis of 6 months or less. Hospice services are provided in the comfort of a patients home or in a skilled nursing facility. Services include but are not limited to skilled nursing care, certified nursing assistants, social worker, chaplain and volunteer services all of which are overseen by our Medical Director, Dr. Nikkalyn DeLaurentis. We also provide medical equipment to assist in the home, medications, and many medical supplies to provide the best care for our patients. In Hospice
